You must COMPLETE and BRING all forms and documents required in this PRE-REGISTRATION FORM. In order to complete your application assistance at the fair, you must provide the information required in the following forms. With this information your application process will be faster and safe.

IMPORTANT: If you have been DETAINED, ARRESTED, HAVE FAILED to pay “CHILD SUPPORT”, and or HAVE FAILED TO PAY YOUR TAXES, you must bring your document records to the fair. Also, if you’ve had a DUI in the last 5 years please bring with you any police or court records.

When you complete these forms, your waiting time is reduced by an hour or longer. At your fair appointment, you will receive the following free services: citizenship application assistance, legal consultation (only on your citizenship process), and educational materials as well as resources for free citizenship classes.
